Students that wish to register for a course after the publicized last day to register must complete this form.
The request must be accompanied by the appropriate registration form (see below) and must include written approval by the course instructor. Students may.
- Print, sign the form and then take a picture of it an email it to email@example.com OR
- Complete the form and email it to firstname.lastname@example.org from their assigned BMCC student email.
- Have the instructor email their consent, from their BMCC staff email account, to email@example.com

- Currently registered for the term and want to add a course: Use a Schedule Change Request form
- Not registered for the current term: Use a Registration form
Students will be notified by email, to their assigned BMCC email account, of the status of their request.
Note: Requests received without course instructor approval will not be processed.

- Summer 2019: June 28, 2019 - August 1, 2019
- Fall 2019: September 28, 2019 - November 15, 2019
- Winter 2020: January 11, 2020 - February 28, 2020
- Spring 2020: April 11, 2020 - May 1, 2020
