Once permission is granted:
Your document must include a notice of the owner of the copyright, year of publication and publisher’s name.
Obtaining copyright clearance can take time so you MUST plan ahead.
If you have not received written permission, you can’t use the copyrighted material. You can’t assume that lack of response means you have permission.
